COVID-19 Reporting Form

As we move into the second half of the school year, we wanted to share another reminder along with a reporting form related to the COVID-19 procedures and information:

  1. As shared within the updated isolation and quarantine guidance from the health department, a reminder that the school and health department recommend that an individual who tests positive for COVID-19 wear a mask on days 6-10, if they are no longer isolating.

  2. We are utilizing a NOSD COVID-19 Reporting Form (see link below) to simplify and streamline reporting. If your child or a household family member has symptoms of COVID-19 and/or tested positive for COVID-19, please complete this form as your first step. Also remember to call your school secretary to report any absence. This simple form will allow a member of our team to reach out to you with your child’s specific return dates.
