Project Saving Smiles Deadline Approaching
The tentative Project Saving Smiles date is Feb 13th - 15th 2023 . The final date will be announced by the nurse once confirmed.
Completed consents must be returned to the nurse by Monday Dec. 19th to be eligible.
Pick -up consent forms from Nurse Shioura in the clinic, Mr Estrada or online.
FREE COVID-19 TESTING FOR STUDENTS AND CAMPUS STAFF
You can register to participate in the district COVID testing programming on campus. The parent/guardian must first give consent. The first test date is November 16, 10am at HAIS and occur bi-weekly thereafter. Only students that have consent will be allowed to test.
To give consent to participate in the COVID-19 testing program, please follow these steps:
- Go to http://www.houstonisd.org/COVIDtesting and click on the “begin” button to start the consent process.
- Enter your child’s student ID and select their school (if you are providing consent for more than one child, you will need to complete the form for each child).
- On the following screen, please enter your child’s date of birth, first name, and last name.
- You will be asked to enter an email address to continue. After entering your preferred email address, you will be sent a link to confirm registration and finalize your child’s information. Please note that the link is time-sensitive and expires in 36 hours. For complete step-by-step instructions, please click here.
HCC Information
HCC FERPA Forms
HCC FERPA Forms allow professors and parents to communicate once and only if students turn completed forms.
The first form is the notary form. This form is used when students are unable to be in person at the HCC Campus Registration Office. Once complete, students submit the form to HCC. Students/parents should submit the completed Notary form to their Grade Level Dean.
Gomez- 9/10
Devaney- 11/12
The second form, the FERPA Release Form, is the traditional form used in non-Covid times. Students complete this form in person at the HCC Campus Registration Office.
