Superintendent's Message
Dear Parents and Guardians:
I want to take a moment to address a few questions that I have received regarding the Daily COVID-19 Questionnaire Form that needs to be completed for each child that attends our schools prior to arrival.
The purpose of this questionnaire is quite important. While we are going to be doing temperature checks upon arrival, please understand that there are other symptoms that we must also take into consideration other than a fever. I understand convenience is important to everyone, but safety measures must supersede.
We are the only district in Hudson County that is offering parents the option to attend school for five days per week for full school days and with that decision, we all must make a commitment to one another to follow these safety measures. This decision was not made lightly. We weighed out the educational benefits and listened to the needs of our staff and families. We put a number of precautionary procedures in place, including the Daily COVID-19 Questionnaire in order to conduct a pre-screening of symptoms prior to students and staff members entering our school buildings.
We made the determination that the window of time to complete the Daily COVID-19 Questionnaire Form would be from 5:00 a.m. to 7:00 a.m. The reason for this is that we must review any questionnaire that has been flagged or students who do not have a completed questionnaire submitted, prior to arrival. Our nurses need time to do this. I have received some messages from families stating that they are too busy to complete it or that the time frame is too early to complete.
I have agreed to extend the time frame from 5:00 a.m. and 7:45 a.m. on a daily basis. The completion of this questionnaire in order for your child or children to enter our school buildings is non-negotiable. The questionnaire, which is accessible via Genesis, can be completed from your phone or a computer/laptop/I Pad in your home. It will take no more than 2 minutes to complete. I thought it would be a good idea to show you what you will be asked on the questionnaire. Please see below:
Section A - Is your child experiencing any of the following symptoms at this time (check boxes that apply):
- Fever
- Chills
- Rigors (shivers)
- Myalgia (muscle aches)
- Headache
- Sore Throat
- Nausea or Vomiting
- Diarrhea
- Fatigue
- Congestion or Runny Nose
Section B - Is your child experiencing any of the following symptoms at this time (check boxes that apply):
- Cough
- Shortness of Breath
- Difficulty Breathing
- New Loss of Smell
- New Loss of Taste
Section C - Please verify if (check boxes that apply):
- Your child has had close contact (within 6 feet) of a COVID-19 infected person
- Someone in your household is diagnosed with COVID-19
- Your child has traveled to an area of high community transmission

As you can see, this questionnaire is not lengthy and does not require you to type in any specific information. Rather, it simply informs you if your child's symptoms preclude them from school that day and allows our nurses to reach out and be sure that your child is feeling better before returning to school. This is not too much to ask as we prepare to educate your children in the safest environment possible.
